When you submit our contact form, you choose what to send us. The form asks for your name, phone number, email address, ZIP code, where you are in the claim process, and a description of what happened.

How Form Submissions Reach Us
Our contact form is delivered by Web3Forms, a form-handling service. Your submission passes through their systems on its way to our email inbox, and is handled under their privacy policy. We do not use Web3Forms for anything other than delivering these messages to us.

We use Google Analytics, a service provided by Google LLC, to understand how visitors find and use our website. This helps us improve the site and learn which pages are most useful.

Children
This website is not directed to children, and we do not knowingly collect information from anyone under 13.
